Job Summary
A publicly traded managed health care company is filling a position for a Telecommute Disease Management Specialist in Rancho Cordova, California.
Must be able to:
- Conduct one-on-one and group based disease management education by telephone
- Conduct brief needs assessment to assist the beneficiary and identify resources needed to assist with the appropriate condition presented by the beneficiary and or group
- Conduct individual needs assessments and provides related resource information to beneficiaries
Qualifications Include:
- Degree or related education for disease specialty
- Two years experience with disease-specific education, either outbound or resource line service or any combination of academic education, professional training or work experience, which demonstrates the ability to perform the duties of the position
- Demonstrated knowledge of disease process and its management, including resource line knowledge
- Ability to motivate patients to change through knowledge and ownership of self-empowerment
- Proficiency in Word, Lotus (notes), and Access
