Freelance Case Manager

Case Management - Manage care with members/patients, their families or significant others and coordinate with the PCP, SCP Social Workers, Home Care Services, Behavioral Health, CBO Intensive Case managers and other community services and all members/patients of the health care team facilitating care across continuums of care episodes.

Review of Specialist Network with members/patients

Review of services: Available in network with members/patients

Review of services needing authorization with members/patients

Review Available Community resources

Drug Screening/review with members/patients Review with ESI (PBM)

Educate members/patients on disease state and recommend interventions to improve outcomes.

Assign members/patients to Telephonic CM follow-up between MD visits

Function as a clinical resource person/educator for the multi-disciplinary health care team on an ongoing basis in order to maximize quality of patient care, effectively manage length of stay and utilization of resources.

NOTES: Telecommuting is allowed. Local Residents Preferred (No Relo)

Requirements

Two plus years Case Management and/or related experience. Current NY or NJ - RN licensure required.
