Telecommute Bilingual Nurses for Home Based Disease Mgmt.

The Disease Management Registered Nurse provides disease management nursing services and care coordination to high risk, high cost Medicaid patients in a geographical region, or telephonically to a commercial population. The RN provides education, counseling and on-demand information to patients who have enrolled in a condition specific program. As needed, the RN will provide program education and coordinate care needs with the patient’s Primary Care Provider. Services are performed in person or telephonically and interventions are recorded in the Population Care Management (PCM) application.

**MUST BE WILLING TO WORK UNTIL 8PM**(FLEXIBLE SCHEDULES AVAILABLE)

**MUST BE WILLING TO WORK ONE SATURDAY PER MONTH

**MUST BE BILINGUAL (SPANISH)

Job Requirements

Minimum Job Qualifications:

Education/Training –

• Associate degree or diploma in Nursing. Bachelor’s degree in Nursing preferred.

Business Experience –

• Experience in a clinical call center preferred.

Specialized Knowledge/Skills –

• Current RN license in good standing.

• 3-5 years recent acute care nursing experience.

• Excellent communication and customer relation skills.

• Excellent clinical documentation and writing skills.

• Excellent critical thinking, decision-making, and problem-solving skills.

• Excellent computer/keyboard/mouse skills.

• Excellent multi-tasking skills with demonstrated ability to simultaneously navigate a computer system and phone system while effectively communicating with patients.

Working Conditions:

Environment (Office, warehouse, etc.) – Office environment; MUST BE ABLE TO GET HIGHSPEED INTERNET VIA CABLE/DSL

Physical Requirements (Lifting, standing, etc.) – Ability to use a computer keyboard, mouse, telephone, and telephone headset.
