The Program Manager is responsible for the development and ongoing management of one or more multi-year programs within a business unit. Program managers typically support business strategies through an integrated portfolio of projects or initiatives. A program manager may have responsibility for a piece of a larger enterprise/regional program.

 

Key functions of this Program Manager position include working with physicians and members to capture documentation surrounding a member's chronic condition.   This position requires extensive outreach to both physician offices and members to help guide predefined behaviors surrounding the member's chronic condition. Essential duties include, but are not limited to:

    * Coordinates and manages on-going programs

    * Ensures program meets its stated objectives; provides subject matter expertise in response to day to day business issues

    * Manages relationships between physician offices and members

    * Performs periodic assessments of program success

    * Performs other duties as assigned.

 

 

Qualified candidates will have the following:

    * RN licensure required.

    * BA/BS degree and 5-7 years of experience in related field or an equivalent combination of education and experience.

    * Demonstrated leadership skills required and ability to be self managed as this is a work from home position.

    * Proven planning and organizational skills to ensure development and maintenance of relationships across organization.

    * Understanding of complex business processes required.

    * Project management experience preferred.

    * Knowledge of physician office dynamics essential.

    * Ability to remove barriers for the member to receive care.

    * Comfortable communicating to both physicians and members over the phone.

    * Excellent knowledge of medical terminology and chronic care management.

    * Basic computer skills required.

    * Knowledge of Medicare's Risk Adjustment program a plus

    * Knowledge of care management a plus.
