Telecommute RN Case Manager
Company is seeking a RN Case Manager to join our team in Melville. In this position, you will acheive optimal benefit and results from the comprehensive management of patients with chronic and/or catastrophic illness, patients who are frail elderly and/or patients whose illness is complicated by challenging psychosocial conditions. Identify patients appropriate for case management. Develop, facilitate and communicate a plan of care in partnership with patient, physicians and various projects.
Ideal candidate will be a graduate of an accredited 2 or 4 year nursing program. Advanced degree is a plus. Current license to practice professional nursing mandatory; certificate as a Case Manager preferred. Minimum of 5 years clinical experience in a high-tech or home health setting. Prior case management and/or managed care experience preferred.
This position offers the potential for a 4 day condensed work week/telecommute option.
