Experienced RN for In Home Care Management
Care Management and Licensed Home Care Provider has an opening for an RN Care Manager to work from home. This position entails assessing client’s status, creating a customized care plan, executing care plan and coordinating referral services, meeting with client in home and/or at appointments, and making suggestions/estimating care costs.
- Must be a Registered Nurse with a BSN
- Must possess at least 2 years of current Health Care Experience
- Must possess at least 2 years of community based Health Care
- Must have verifiable clinical competency and provide evidence of ongoing educational training
