Field Care Manager for Social Worker or Nurse

Company has an opportunity for a Field Care Manager to assist with in-person assessments, interventions, and provide follow ups for participants at their homes.

• Must have knowledge of care/case management principles
• Must be able to collect and analyze assessment information and determine intervention strategies
• Must possess exceptional ability to process information electronically
• Must be able to prioritize, meet expectations, and communicate effectively
• Must be an RN with at least an Associate’s Degree in Nursing and 3+ years experience; OR a BA/BS in Social work with 5+ years of case management experience; OR possess a Master’s Degree in a related field with 3+ years of care/case management experience
• Must reside in Pennsylvania, and possess a valid PA Driver’s license and reliable transportation
• Must be able to meet participants at their homes
• Must possess a separate office space with a lockable door to ensure patient privacy while working
• Must possess a high speed internet connection: laptop, Blackberry and printer are provided

Location: Work at Home/Pennsylvania
