Job Summary

To support members in achieving diabetic reversal and overall health improvement, the California Licensed Bilingual Physician Assistant will provide remote care management, conduct health assessments, and collaborate with a team of healthcare professionals.

Key responsibilities

• Engage with a collaborative team of healthcare professionals, including health coaches and physicians
• Conduct health assessments and collaborate with members' primary care providers to assess and treat symptoms
• Provide patient education and manage a high-risk population, focusing on social determinants of health

Required qualifications

• California state Physician Assistant license, preferably licensed in multiple states
• Bilingual in English and Spanish
• 3-5 years of experience as an advanced practice provider
• Clinical experience working with diverse populations, including underserved groups
• Board certification in family or internal medicine
