California Licensed Care Manager
Job Summary
Providing ongoing remote care management, the full-time California Licensed Care Manager will monitor patient vitals, develop individualized care plans, and deliver education to improve adherence and outcomes while collaborating with healthcare teams.
Key Responsibilities
- Monitor and interpret patient vitals, assessments, and alerts within the care management platform
- Develop, implement, and adjust individualized care plans addressing clinical and social needs
- Coordinate with physicians and community resources to ensure seamless care and improve patient engagement
Required Qualifications
- Active and valid registered nurse (RN) license in the state of California
- Two (2) to five (5) years of direct patient care experience in medical-surgical or home care
- Verbal and written proficiency in Vietnamese
- Bachelor's degree in nursing or a closely related healthcare field
- Ability to work independently in a remote, technology-enabled environment
