Cardiac Care Nurse Practitioner

Job Summary

A company is looking for an Advanced Practice Provider, Nurse Practitioner or Physician Assistant to oversee patients in a virtual cardiac rehab program.

Key Responsibilities:

• Provide Evaluation and Management telehealth visits to gain referrals for RPM and CCM programs
• Serve as a physician champion/liaison for the remote cardiovascular risk prevention program
• Engage with leadership at health systems and educate them on the benefits of the program

Required Qualifications:

• Nurse Practitioner or Physician Assistant with a Master's degree or accredited program graduation
• Board certified and licensed in Massachusetts and New York
• Experience in cardiac care
• Experience in remote patient monitoring and chronic care management preferred
• A highly compassionate and empathetic individual
