Job Summary
A company is looking for a Care Advisor Researcher.
Key Responsibilities:
- Research and partner with community organizations to provide resources to clients
- Document resources on shared team site and create personalized action plans
- Communicate effectively with team and cross departments
Required Qualifications:
- Bachelor degree in Psychology or Social Work preferred
- Strong background in geriatric case management or coordination experience required
- Experience in Social Services, Care Coordination, Healthcare Navigation, or related fields preferred
- 1 year of relevant experience in customer service or similar role preferred
- High Volume Call Center experience is a plus
