Job Summary
A company is looking for a Care Coordinator to facilitate patient care and communication among interdisciplinary teams.
Key Responsibilities
- Support patients in scheduling provider visits
- Ensure proper documentation of all patient contacts
- Answer patient calls and appropriately transfer escalated patient issues
Required Qualifications
- High School Diploma / GED (or higher)
- Experience with escalated customer service issue resolution
- Knowledge of Microsoft Office products and proficiency in Microsoft Excel
- Phone/Fax system experience
- EMR experience, with Athena & Epic preferred
