Job Summary
A company is looking for a Care Coordinator to assist families in managing their healthcare needs.
Key Responsibilities:
- Assist families in organizing care plans and managing healthcare-related tasks
- Collaborate with Care Advisers and Care Manager to provide comprehensive support
- Support other service team members and contribute to product and service improvement
Required Qualifications:
- Bachelor's degree in Social Work, Public Health, Healthcare Administration, or a related field
- 4-6 years of healthcare experience, including case management and coordination
- Experience in developing and facilitating care plans
- Ability to manage multiple care projects with compassion
- Proficiency with macOS systems, GSuite, Slack, and 1Password is preferred
