Job Summary
A company is looking for a Care Coordinator - Clinical Operations.
Key Responsibilities
- Conduct outreach services to complete Social Determinants of Health (SDOH) and Medication Adherence screening assessments
- Make referrals to in-house clinical social work or pharmacy teams based on assessment results
- Document all inbound and outbound inquiries and resolve them according to client direction
Required Qualifications
- High School Diploma or equivalent
- 1+ years of experience in accessing community resources preferred
- Bi-lingual Spanish and English language proficiency required
- Knowledge of managed care and/or medical experience preferred
- Ability to navigate multiple technology systems
