Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Perform outreach to members to assist with care plans and connect them to resources
  • Serve as a point of contact for member and provider inquiries, explaining procedures and protocols
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements

Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Knowledge of health plan activities and community resources
  • Ability to work Eastern Time Zone hours, Monday - Friday, 8 AM - 5 PM
  • Compliance with all policies and standards is necessary
