Job Summary
A company is looking for a Care Manager to support members in improving their health and managing chronic conditions.
Key Responsibilities:
- Develop trusting relationships with members to facilitate behavior change and health management
- Coordinate care and provide educational resources tailored to members' needs
- Create individualized health plans with specific goals and action steps
Required Qualifications, Training, and Education:
- College degree in a health-related field preferred
- Current and unrestricted OTA license/certification required
- Minimum 1-3 years of clinical experience with patient assessments
- Experience with Motivational Interviewing techniques and working with the Medicare population
- Culturally sensitive and competent for assigned membership
