Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and develop personalized care plans
- Coordinate care between members, families, and healthcare providers
- Monitor member progress and provide education on healthcare options
Required Qualifications and Education
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Active RN license in Arizona and Washington, with a California RN license
- Experience in coordinating care and accessing community resources
- Knowledge of social determinants of health and healthcare regulations
