Job Summary
A company is looking for a Care Manager RN - National Remote.
Key Responsibilities
- Coordinates patient-centered care and develops safe discharge plans
- Facilitates communication among the multidisciplinary healthcare team and patients
- Manages utilization of resources to reduce length of stay and improve patient outcomes
Required Qualifications
- Associate's Degree (or higher) in Nursing
- Current, unrestricted RN license in the state of residence
- 3+ years of experience in a hospital, acute care, or direct care setting
- Intermediate level of ability to type and navigate a Windows based environment
- Preferred: Bachelor of Science in Nursing (BSN) or higher
