Case Manager RN (NV & CA)

Job Summary

A company is looking for a Case Manager RN (RN in NV & CA).

Key Responsibilities

• Facilitate the delivery of healthcare information and determine eligibility for benefits
• Develop and implement health strategies and programs to promote member wellness
• Coordinate care and advocate for services to meet comprehensive health needs

Required Qualifications

• Active and unrestricted RN licensure in CA and NV compact
• Minimum 3+ years of clinical practical experience, preferably with Medicare members
• Minimum 2+ years of case management, discharge planning, or home health care coordination experience
• Associates Degree or Nursing Diploma required; Bachelor's Degree preferred
