Job Summary
A company is looking for a Chronic Care Management Care Coach in the United States, Pacific Time Zone.
Key Responsibilities:
- Conduct telephonic and virtual care management for patients with chronic diseases
- Collaborate with healthcare providers and community resources to optimize patient care
- Educate patients on chronic disease management and support in behavior modification interventions
Qualifications:
- Bachelor's degree in Nursing, Social Work, or related healthcare field
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred
- Minimum of 2 years experience in chronic care management or related healthcare setting
- Strong knowledge of chronic disease states and management
- Current CPR certification
