Clinical Care Coordinator - CO LAC

Job Summary

A company is looking for a Clinical Care Coordinator in the Mountain Time Zone.

Key Responsibilities:

• Provide consultation to providers and consumers on benefit information, safety issues, and care coordination
• Manage behavioral health services, including crisis assessment and intervention
• Process pre-authorizations, conduct patient assessments, and coordinate transitions between care areas

Required Qualifications:

• Current, unrestricted licensure as a Licensed Addictions Counselor (LAC) in Colorado or LPC, LMFT, or LCSW with psychiatric and substance use treatment experience
• 4+ years of experience serving individuals with behavioral health conditions
• 1+ years of experience with local behavioral health providers and community support organizations addressing social determinants of health (SDoH)
• 1+ years of experience with MS Office, including Word, Excel, and Outlook
• Must be able to work Monday through Friday 9:30am - 6:00pm Mountain Time Zone
