Job Summary
A company is looking for a Clinical Care Coordinator (remote) in Oregon.
Key Responsibilities
- Provide integrated, trauma-informed care coordination services to members with complex behavioral health needs
- Develop individualized plans of care and facilitate communication between members and their support systems
- Participate in interdisciplinary team meetings and provide technical assistance to community partners
Required Qualifications, Training, and Education
- Master's degree in social work, psychology, sociology, or a closely allied field
- Licensed Clinical Social Worker or equivalent Oregon licensed healthcare profession
- Five years of experience in mental health and knowledge of DSM and ASAM criteria
- Experience with programs such as EASA, IIBHT, or coordinating between levels of care
- Bilingual in Spanish preferred
