Clinical Care Specialist - RN

Job Summary

A company is looking for a Clinical Care Specialist - Remote (US).

Key Responsibilities:

• Maintain and manage patient census for chronic care management
• Coordinate care plans and conduct care coordination with patients and their care team
• Assess and close clinical and non-clinical gaps in patient care

Required Qualifications:

• RN license in applicable state with active compact license
• Five years of nursing experience
• Experience serving chronically ill patients preferred
• Telephonic case management or care coordination experience preferred
• Compassionate and friendly demeanor
