Clinical Case Management Analyst

Job Summary

A company is looking for a Clinical Case Management Analyst to support clinical analytics and manage operational requirements of clinical case management.

Key Responsibilities

• Conduct provider and member outreach for clinical case management and disease state management
• Document and maintain case assessments and care management plans in electronic patient management systems
• Collaborate with Clinical Analytics staff to develop and implement clinical initiatives

Required Qualifications

• Active certified pharmacy technician license in the state of employment preferred
• Bachelor's degree in a relevant healthcare field preferred
• 2+ years in the healthcare industry, including experience in Pharmacy Benefit Management or Managed Care
• 2+ years working with pharmacy data management systems or medical data analytics
• Proficient in Microsoft Office Suite and database management, including Access
