Clinical Pharmacist - TX Licensed
Job Summary
A company is looking for a Clinical Pharmacist for a remote call center position in Texas.
Key Responsibilities:
- Conduct telephonic medication consultations and annual medication reviews
- Assess patients for medication adherence barriers and develop improvement plans
- Document clinical consultations and participate in staff development opportunities
Required Qualifications:
- Doctor of Pharmacy degree or equivalent experience
- Active, unrestricted pharmacy license in Texas or ability to obtain licensure in multiple states
- 3+ years of Clinical Pharmacy Practice experience
- 1+ years of Call Center experience
- Experience with Medicare STARs & HEDIS Measures
