Community Health Worker Supervisor
Job Summary
A company is looking for a Supervisor, Community Health Worker to lead a team of community health workers in addressing health disparities.
Key Responsibilities:
- Recruit and engage community health workers (CHWs) through local community-based organizations (CBOs)
- Lead and support a team of CHWs in providing advocacy and social support to clients
- Analyze performance reports to ensure effective service delivery and integration with care teams
Required Qualifications, Training, and Education:
- Leadership experience with a non-profit or community-based organization
- Experience in clinical social work or case management
- Strong understanding of healthcare and social service systems
- College degree; MSW or MPH preferred
- Passion for addressing health inequality and community resources knowledge
