Complex Discharge Planner - NV Licensed

Job Summary

A company is looking for a Complex Discharge Planner I (Nurse or Social Work) to facilitate transitions from acute care settings for high-risk members.

Key Responsibilities

• Assess, plan, and coordinate safe and effective transition plans
• Collaborate with internal and external teams for clinical case review and planning
• Identify and address barriers to reduce length of stay and readmissions

Required Qualifications

• Bachelor's degree or graduate degree in nursing or social work
• 3+ years of nursing or social work experience required
• 1+ years of care coordination or discharge/transition planning preferred
• Current state's RN license, LMSW, or LICSW required
• Knowledge of public health, home health, or acute care transitions preferred
