Job Summary
A company is looking for a Care Manager (RN) Diabetes.
Key Responsibilities
- Develops and facilitates complex care management activities and personalized care plans for members
- Coordinates care between members, families, and providers to ensure appropriate person-centered services
- Provides education to members and families on disease processes and healthcare options
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Experience in diabetes education strongly preferred
