Job Summary
A company is looking for a Health Risk Assessment Nurse (Remote - AZ).
Key Responsibilities
- Complete initial and annual Health Risk Assessments and create individualized care plans
- Utilize risk stratification tools to classify members' acuity levels and identify gaps in care
- Collaborate with the Interdisciplinary Care Team to ensure continuity of care and educate members on preventive measures
Required Qualifications
- 2 years of nursing experience in a healthcare setting
- Associate's Degree in Nursing or related field
- Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Arizona
