Housing Specialist - Community Health Worker

Job Summary

A company is looking for a Housing Specialist - Community Health Worker to oversee housing initiatives for Medicaid members in Florida.

Key Responsibilities

• Research and maintain a list of available housing opportunities in the region
• Develop individualized housing plans and assist members in their housing search
• Conduct follow-up meetings with members after housing placement to address issues

Required Qualifications

• High School Diploma/GED or higher
• 3+ years of experience managing needs of complex populations (e.g., Medicare, Medicaid)
• 3+ years of experience in a community health setting
• 1+ year of experience with the homeless population and housing
• Must reside in the state of Florida
