Job Summary

Managing a moderate-to-high complexity clinical caseload, the full-time LCSW Licensed Clinical Care Navigator will independently coordinate care for patients with Substance Use Disorder, ensuring alignment with clinical intent and operational execution while working remotely.

Key Responsibilities
  • Manage a caseload of approximately 100-125 complex patient cases requiring coordinated referrals and follow-up
  • Coordinate care across clinical experts, operational teams, and external partners to enhance patient outcomes
  • Surface insights to improve clinical navigation workflows and serve as a resource for peers on best practices
Required Qualifications
  • 4+ years of clinical experience in Substance Use Disorder and Behavioral Health with an LCSW, LCPC, LSW, or LPC License
  • At least 3+ years of direct patient care or coordination experience
  • Substance Use Disorder certification is strongly encouraged
  • Preferred: Spanish speaking
  • Preferred: Direct patient experience in broader behavioral health areas, including eating disorders and acute mental health needs
