Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Clinical Case Manager Behavioral Health DSNP.

Key Responsibilities

• Conduct comprehensive assessments of members' needs and eligibility for services
• Utilize clinical judgment to develop care plans and coordinate services for members
• Provide crisis intervention and follow-up care to ensure members receive appropriate treatment

Required Qualifications

• 3 years of direct clinical practice experience post-master's degree
• 3 years of knowledge in mental health and substance abuse disorders
• 3 years of experience using Motivational Interviewing and technology skills
• Valid unrestricted independent professional behavioral health clinical license
• Must be available to work Monday through Friday, 8:00am to 5:00pm
