Job Summary
To enhance patient care coordination, the full-time Licensed Care Coordinator, Behavioral Health will manage a moderate caseload by collaborating with clinical teams, ensuring compliance with treatment plans, and facilitating care transitions while working remotely.
Key responsibilities:
- Coordinates patient care by collaborating with clinicians to review and improve treatment plans and make referral recommendations
- Ensures quality care by documenting treatment plan progress and advocating for patients during care transitions
- Manages patient data and records by compiling information and conducting quality management studies and audits
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
- Licensure as a Clinical Social Worker or Mental Health Counselor in Oregon and Washington within six months of hire
- National Provider Identifier required at hire
- Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
