Licensed Care Manager
Job Summary
A company is looking for a Care Manager Transition of Care.
Key Responsibilities
- Evaluate member needs through post-discharge assessments and medication reconciliation
- Develop care/service plans and collaborate with healthcare teams to support member transitions
- Facilitate education and resources for members to promote wellness and improve care quality
Required Qualifications
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
- 2 - 4 years of related experience in care management
- Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
