Job Summary

A company is looking for a Maternal Child Health Clinical Coordinator RN in Pennsylvania, with remote work options available.

Key Responsibilities

• Develop strategic relationships with community organizations providing maternal and child health services
• Manage quality improvement projects and ensure program efficiency for the Healthy First Steps program
• Oversee care coordination for high-risk members and facilitate transitions of care to community resources

Required Qualifications

• Current and unrestricted Pennsylvania RN License
• 4+ years of Maternal and/or Pediatric clinical experience
• 3+ years of care coordination and/or utilization management experience
• 2+ years of experience working in Medicaid
• Intermediate level of proficiency with Microsoft Office Suite
