Job Summary
A company is looking for a Social Worker to coordinate socioeconomic and psychosocial services for members in a remote setting.
Key Responsibilities
- Coordinate psychosocial services and assist members with medical resource utilization
- Conduct needs assessments to identify barriers and recommend resources
- Collaborate with interdisciplinary teams to develop member-specific care plans
Required Qualifications
- LCSW, MSW, and licensed in Missouri and Illinois, eligible for licensure in other states
- Minimum of 2 years of clinical experience
- Experience in a managed care environment preferred
- Computer proficiency
