New York Licensed Care Manager

Job Summary

A company is looking for a HARP Care Manager to plan and manage behavioral and physical care for members.

Key Responsibilities

• Advocate and educate members on services and health benefits
• Conduct assessments and develop care plans in alignment with treatment goals
• Coordinate care among providers and community resources to maximize health outcomes

Required Qualifications

• NYS RN or relevant mental health license (LCSW, LMSW, LMFT, LMHC, LPC, licensed psychologist)
• 3 years of experience in a mental/behavioral health or addictions setting
• Strong interpersonal and assessment skills
• Experience managing large caseloads in a fast-paced environment
• Proficient in using electronic documentation systems and Microsoft Excel
