Job Summary

A company is looking for a Bilingual Pediatric Care Manager to plan and manage behavioral and physical care for pediatric members and their families.

Key Responsibilities

• Plan and manage pediatric medical and behavioral care across the continuum of care
• Develop member-centric care plans in collaboration with families and healthcare teams
• Document all encounters and update care plans based on member needs and goals

Required Qualifications

• NYS RN or LCSW, LMSW required
• 1 year of pediatric clinical field experience, including work with child welfare and medically fragile populations
• Proficient in navigating electronic documentation systems
• Ability to collaborate with multiple stakeholders for care coordination
• Strong interpersonal skills, particularly in crisis situations involving children and families
