Job Summary

A company is looking for a Patient Care Coordinator - Remote.

Key Responsibilities

• Serve as a liaison between members and healthcare providers, facilitating access to care and resources
• Coordinate interdisciplinary care team meetings and manage member care plans
• Assist members in navigating healthcare systems and achieving preventive care goals

Required Qualifications and Education

• High School diploma or GED required; Medical Assistant preferred
• One year of managed care experience preferred
• Equivalent clinical work experience considered
• Proficient in computer skills, particularly in EzCap, Word, and Excel
• Knowledge of Medicare and Medi-Cal benefit structures preferred
