Job Summary
A company is looking for a Population Health Registered Nurse to improve patient health outcomes through telephonic outreach and care coordination.
Key Responsibilities
- Conduct telephonic outreach for post-discharge assessments and chronic disease management
- Educate patients on preventative health screenings and medication adherence
- Coordinate care between healthcare services to enhance patient outcomes
Required Qualifications
- 2+ years of experience in population health, case management, or primary care with complex chronic conditions
- Nurse Licensure Compact (NLC) required
- Highly culturally competent with diverse populations
- Familiarity with EMR systems and technology for virtual care delivery
- Bilingual in Spanish is a bonus
