Registered Nurse Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members in Hudson and/or Essex County, NJ.
Key Responsibilities
- Evaluate member needs and develop personalized care plans
- Coordinate care between members, families, and providers to ensure access to services
- Monitor member progress and update care plans as necessary
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience
- Registered Nurse (RN) State Licensure or Compact State Licensure required
- Must reside in Hudson and/or Essex County, NJ
