Registered Nurse Care Manager
Job Summary
A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
- Coordinate with providers and community resources to ensure members receive person-centered care
- Monitor member status and update care plans as necessary to meet goals and address unmet needs
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Must have KY or compact license and NY license
- Ability to obtain other single state licenses
