Job Summary
A company is looking for a Patient Engagement Specialist, Registered Nurse.
Key Responsibilities
- Conduct telephonic outreach to patients for post-discharge assessments and address their clinical and social needs
- Educate patients on preventative health screenings and manage chronic disease education
- Coordinate care between healthcare services to enhance patient outcomes and collaborate with internal teams
Required Qualifications
- 2+ years of experience in case management or primary care with a focus on population health
- Nurse Licensure Compact (NLC) required
- Active RN license(s) in NV, MI, MN, OR, CA, MA, CT, or IL preferred
- Experience with motivational interviewing techniques
- Bilingual in Spanish is a bonus
