Job Summary

A company is looking for a Clinical Registered Nurse - Remote.

Key Responsibilities

Provide telephonic support and coordinate with healthcare teams to manage participant care
Identify educational needs and facilitate care plans for participants
Ensure continuity of care and communicate effectively with primary care providers and other team members

Required Qualifications

Current RN license for the relevant state and ability to obtain licensure in other states
High School diploma and a Diploma, Associate's, or Bachelor's Degree in Nursing (BSN preferred)
Minimum of 3 years recent clinical experience
Basic knowledge of physiology and chronic disease management
Basic computer skills and familiarity with Windows operating systems
