Job Summary

A behavioral health solutions company needs applicants for an opening for a Remote Behavioral Health Case Manager.

Core Responsibilities of this position include:

  • Utilizing the components of the case management process
  • Promoting and providing quality service delivery and effective care transitions planning
  • Conducting telephonic assessments with the members to gather information

Skills and Requirements Include:

  • Conducts on-site and telephonic communication
  • Has a current, unrestricted state license issued by a state or territory of the United States
  • 3-5 years of direct clinical care experience with patients
  • Strong clinical and case management skills
  • Excellent verbal and written communication and interpersonal skills
  • Strong computer and keyboarding skills
