Job Summary

A healthcare technology company has an open position for a Remote Bilingual Spanish Care Coordinator.

Core Responsibilities Include:

• Using proprietary care plan to guide patient interaction and direction
• Creating and maintaining comprehensive, proprietary patient care plan
• Utilizing chronic condition specific scripts to guide conversation

Must meet the following requirements for consideration:

• Speaks fluent Spanish and English
• Minimum 1-year experience in homecare or clinical setting
• Knowledge of medical terminology
• Minimum 1-year experience in Chronic Care Management
• Medical Assistant certification or higher (LPN, RN, BSN, etc.)
• Critical thinking skills
