A custom resources platform for patient management has an open position for a Remote Bilingual Spanish Patient Monitoring Licensed Practical Nurse in San Antonio.
Individual must be able to fulfill the following responsibilities:
- Remotely monitor the patient's blood pressure, glucose levels, etc
- Abide by the plan of care and orders of the practice
Must meet the following requirements for consideration:
- Ability to provide prevention and intervention for multiple disease conditions through motivational coaching
- Experience using Electronic Health Records
- Licensed Nurse with a current active license in the state of residency
