Job Summary
A population health management company is filling a position for a Remote Bilingual Spanish Population Health Case Manager in Las Vegas.
Core Responsibilities of this position include:
- Conducting meaningful outreach, providing education, and driving member enrollment
- Creating service plans to guide engagement based on member diagnoses, results of screenings, as well as care gaps
- Building measurable goals for members to develop skills and/or strategies for managing challenges and triggers
Skills and Requirements Include:
- Up to 50% travel (locally) and, therefore, requires the employee has reliable transportation
- Bachelor’s Degree required
- Related experience within behavioral and/or public health
- Complete short-term projects within given deadlines
- Document meticulous member records
- Manage a complex calendar to reach individual and team goals
