Job Summary
A behavioral health care and human services provider has a current position open for a Remote Care Advocate in Columbus.
Must be able to:
- Partner with participants in their homes, communities & through the complicated paths of the healthcare system
- Work with patients to achieve their goals, identify new needs & problem-solving by offering creative solutions to any barriers to care
- Coordinate with patients & care teams for care planning & delivery by organizing schedules, managing appointments, referrals, etc
Must meet the following requirements for consideration:
- Minimum 1 year as a MA
- Experience in family medicine, addiction medicine, or mental health
- Bachelor’s degree in psychology, public health, social work, or equivalent
- OR 5+ years experience in health care such as patient advocacy, behavioral health, primary care, care coordination, etc
