Job Summary

An insurance company has an open position for a Remote Care Management Associate in the Two Rivers Region.

Candidates will be responsible for the following:

• Screening patients using targeted intervention business rules and processes
• Monitoring non-targeted cases for entry of appropriate discharge date and disposition
• Being responsible for initial review and triage of Care Team tasks

Position Requirements Include:

• Travel 0 - 10%, some travel to the Louisville office may be required for meetings and training
• High School Diploma or G.E.D
• Experience or detailed knowledge of the Foster Care and juvenile justice systems, Adoption Assistance,
• 2-4 years' experience as a medical assistant, office assistant
• Computer literacy and demonstrated proficiency
• Suite applications, including Word and Excel
